What’s the offer?

In case you missed our email, here are the details again.
From 1 December 2018 to 31 January 2019, any customer who purchases our Personal Health plan will receive a free Realise Health Assessment Plus with home collection kit (RRP. £99.00). This offer applies to any individual customer who is new to AXA PPP healthcare and comes to us via an intermediary.

What is a Realise Health Assessment Plus?

Good health isn’t just about managing ill health and treating disease – it’s about preventing it. To get customers on the road to managing their health, they can use the assessment to give health insights that are easy to act upon.

How does the individual access the offer?

Once the individual’s plan has been finalised, redemption is very easy. We will email the member mid January/mid February 2019 (depending on when their plan starts) with full details. All they need to do is visit www.axaactiveplus.co.uk/realise-health-assessment-plus and follow the simple instructions.
Please note that your current commission structure will remain unchanged but we hope that this offer will help you promote AXA PPP healthcare over the coming months¹. If you have any questions, please contact your dedicated account manager.

Terms and Conditions

• The new members Personal Health plan must start with a date between 1 December 2018 and 31 January 2019.
• The offer is not open to group leavers.
• There is one free assessment Realise Health Assessment Plus with home collection kit per plan.
• The individual must be a current AXA PPP healthcare member with a Personal Health plan at the time of redeeming this offer.
• The voucher must be redeemed within 3 months of the date issued.
• The offer is non transferable and cannot be substituted for a cash alternative.
• If the individual wants to upgrade their blood test collection method from a home collection kit, to a clinic visit or nurse home visit, this can be purchased at an additional cost to the individual member. See the ActivePlus website for more details.
